Description

Hudson River HealthCare (“HRHCare”) is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. HRHCare is currently seeking a full-time Hepatitis C Linkage to Care Specialist/ Data Specialist which will be housed at our Shirley location.

Essential Duties and Responsibilities of a Hepatitis C Linkage to Care Specialist/ Data Specialist:

  • Work with Community Based Organizations (CBOs) to set up HCV screening services, provide community education and outreach and act as a liaison with CBOs to link HCV positive patients to care
  • Participates in case conferences, as well as other agency or community-based meetings or trainings
  • Educate providers and other staff about HCV testing, benefits of diagnosis and importance of linking to care
  • Develops linkages and internally refers patients for additional medical services, including treatment
  • Facilitates enrollment in HCV Benefits and Entitlement programs
  • Works cooperatively and collaboratively with entire HCV case management, Medical and Behavioral Health Provider Teams
  • Responsible for program data entry and review
  • Works with Program Manager to create and monitor appropriate data reports
  • Maintains data and case records as required and prepares necessary reports

Minimum Education Requirement:

  • Associate's degree

Preferred Education Requirement

  • Bachelor's degree

Minimum Work Related Experience: 0 - 1 years

Preferred Work Related Experience: 2 - 4 years

Preferred Specialized Skills & Knowledge: Bi-lingual, Spanish speaking
