Hudson River HealthCare (“HRHCare”) is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. HRHCare is currently seeking a part-time Peer Adherence Educator to join our team in Poughkeepsie.

The position is a part-time position, 14 hours per week at our Poughkeepsie, 75 Washington St. site providing adherence education and emotional support to HIV positive patients in our HIV primary care and case management programs at Hudson River HealthCare.  The Peer Adherence Educator works with the HIV Specialist and support staff to reduce barriers to healthcare for patients, assess and educate patients on the importance of medical adherence and harm reduction, escort patients to appointments as needed, and complete referrals and follow up contact with patients to encourage retention in care.  The peer shares life experience to provide strategies to patients to assist in coping with living with HIV.

Essential duties and responsibilities of the Peer Adherence Educator:

  • Conducts adherence assessments with individuals living with HIV to identify and reduce barriers to care
  • Refer patients to community resources
  • Implements evidence-based strategies
  • Provides support and follow-up on patient needs
  • Provides harm reduction and adherence education

Qualifications: High School diploma/GED, willingness to share HIV+ status life experiences as a teaching method, knowledge of HIV disease and treatment, demonstrate good verbal and writing skills, initiative, self direction, enthusiasm, cultural sensitivity, ability to interface with community agencies.  Bilingual Spanish/English speaking preferred.
